Depending on the model, the air inlet valve lets in a weak stream of air, either into the radiator or into the ceiling. The cold air from the street mixes with the warm air in the apartment. As a result, the temperature inside always stays comfortable and fresh air enters the house. You will be warm even if it is -50 outside.

The easiest option is to put the vent damper on the balcony and open one window for micro-ventilation. The second option is to install a Norvind lite ventilator in the opposite corner of the balcony. There are several other options, including extending the pipe, but you should consult a home ventilation specialist.
